Professor David Colin Thome OBE Until recently the National Director of Primary Care at the Department of Health (DH), a post he held for 6 years, David worked previously as a GP at the Castlefields practice in Runcorn, which was one of the first to offer a practice-based chronic disease management service focused on improving quality and reducing hospital utilization
