Transforming Care - Launch of the IHS Care Co-ordination Toolkit

 

Many patients, particularly those with complex needs, who are admitted to hospital in an emergency, often could have been managed more effectively, thereby avoiding a crisis intervention.   NHS England recently asked the NHS to improve the coordination of care for people in later life.   Care coordination can help address this challenge – Integral Health Solutions has produced a Care Co-ordination tool-kit.
