Connecting Care

 

Connecting Care, Cheshire – We were commissioned by the Connecting Care Board, Central Cheshire, to undertake a strategic review of the Connecting Care programme to determine whether there is a alignment across the respective strategic and operational plans in terms of objectives and shared vision which give coherence to a shared strategy and a shared common purpose across the Connecting Care partners.

The Connecting Care Board is make up of over 9 organisations including: South Cheshire CCG, Vale Royal CCG, Mid Cheshire Hospitals NHS Foundation Trust, East Cheshire Hospital NHS Trust, Cheshire West and Chester Local Authority, Cheshire East Local Authority, Cheshire and Wirral Partnership NHS Foundation Trust, North West Ambulance Service and representatives from GP federations.

The output from this strategic review is a detailed report setting out the assessment against a model of integration, highlighting areas of misalignment with guidance as to what further work is required to secure alignment.  Flowing from the diagnostic assessment, we consolidated our findings into a set of 6 key themes – with a best practice blueprint for each theme, supported by a set of detailed, prioritised recommendations for implementation.
